By submitting your transport request, you accept the following responsibilities:

    - Ensure on-time arrival of the animal(s) at the originating airport by the sender and on-time pick-up at the destination airport by the receiver. Any required ground transportation is the responsibility of the sending and receiving parties. ARF volunteer pilots transport animals from airport to airport only
    - Provide the animal(s) with a collar, leash and a size-appropriate, airline-approved style crate. Exact crate dimensions must be provided on this form for the transport to be considered as not all crates can fit in all planes
    - Provide a valid health certificate and proof of rabies vaccination for each animal(s) being transported, and that all other state import requirements will be followed
    - Prepare the animal(s) for the flight as follows: 1) Animals over 3 months old must receive no food and limited water within 8 hours before flight, 2) Must have been walked and allowed to urinate/defecate prior to the flight. Animals may not be sedated
    - Seek ground transportation options, which are less impacted by weather and therefore more reliable, and notify ARF immediately if ARF is no longer needed for transport
    - For longer transports, establish "standby foster" arrangements at intermediate locations along the transport route in the event the transport must be aborted due to weather, mechanical or other unknown events All necessary paperwork for the animal(s) as well as the crate must be provided to the pilot upon departure of the flight or the flight will be canceled. The safety and well-being of the animal during the transport is our top concern.
